Individual Intake Appointment
1 hr
An individual intake appointment is an initial one-on-one meeting between a client and a clinician to discuss the client’s concerns, current symptoms, and reasons for seeking services. The clinician gathers information about relevant personal, family, medical, mental health, substance use, and treatment history while exploring current functioning, strengths, supports, and goals. The appointment also includes a review of confidentiality and its limits, an assessment of safety concerns, and an opportunity for the client to ask questions. Together, the client and clinician identify initial priorities and discuss recommendations and next steps for care. Many insurances accepted.
