What is your typical process for working with a new customer?
Clients typically make first contact by phone, where we determine what's going on, and how urgently we need to start. We work with you to find the most convenient time to schedule the initial appointment, and cover any questions you may have about what will happen and potential costs.
What education and/or training do you have that relates to your work?
Physical Therapists require either Masters or Doctoral degrees from accredited Universities or Colleges, then must pass a rigorous Board Certification to become licensed. 20 hours of continuing education must be done and submitted to the state in order to maintain licensure.

Prices are listed on our website for many of our cash packages. Custom programs can be developed depending of the complexity of the condition or number of sessions anticipated to resolve the condition.

How did you get started in this business?
Form and Function exists to provide patients an alternative to homogenized/generic physical therapy that health insurances control.
We help you recover at the pace you want to recover, with experts in the field, not volunteers or students, or factory-like high volume therapy. Individual attention is at the core of our treatment.
What types of customers have you worked with?
We are primarily a sports rehabilitation center, but also work with adults recovering from injury/surgery, back aches, concussion, foot and ankle pain, carpal tunnel, headaches, arthritis, motor vehicle accidents, etc..
We've been at this for 15,000 patients, so there's very little we haven't seen. And if we can't help you, we have created relationships with providers from all disciplines to help you get your needs met.
